Proclamation Request Form

This form must be completed and submitted to the Clerk of the Town of Essex by all persons and organizations requesting that Essex Town Council pass a proclamation. Proclamation requests must be submitted by 2:00 p.m. on the Tuesday prior to the scheduled meeting. Please refer to our online Council Calendar for scheduled Council meetings.

Draft wording for the proclamation may be uploaded at the end of this form or may be submitted by separate email to clerks@essex.ca.

The personal information on this form is collected under the authority of the Municipal Act. The information is used for the purpose of processing the Proclamation Request Form. Questions about this collection of information can be made to the Clerk at 519-776-7336.
